Name: San Bernardino District, CFWC, Nurses Scholarship Fund.

Purpose: This fund shall be established for the purpose of granting scholarships to qualified students pursuing a career as a registered nurse.

Application: Applications should be available from the college's Guidance Office. Completed applications should be returned there, or mailed to Nancy Nelson, Scholarship Chairman, 1601 Paloma, Barstow CA 92311..

Qualifications: To qualify for this scholarship, any applicant must:

1.     Be a legal resident of the State of California for one year, with preference going to residents of
        the County of San Bernardino.

2.     Be accepted into an accredited School of Nursing in California.

3.     Submit a Transcript of college grades and GPA to date of Course completed, and a work-in-progress report
        of any Courses applicant is currently taking. A SPA of 3.0 to 3.5 Is required to qualify.

4.     Attach a statement of at least two hundred words, including the following:

      A.    Why I wish to continue my education.

      B.    My future education or vocational plans.

      C.    My reason for applying for a scholarship.

5.     Submit a financial statement including household size, estimated income for the year,
        and monthly budget of expenses. Explain circumstances that support the Application.

There is no restriction as to age, race color, sex, or creed.

The San Bernardino District Nurses' Scholarship shall be disbursed in allocation, not to exceed $500. Applications must be received by the the District Scholarship Chairman on or before March 15, 2010. Announcement of the recipients will be made at the San Bernardino District Convention in April. The money will be disbursed to the accepted school after the chairman has received official notification from the Registrar's Office that the student is enrolled in their nursing program as a full-time nursing student.
